Evacuation of disabled people from multi-storey residential building

On the fifth (top) floor of a 40-year-old apartment building without an (evacuation) elevator, with one staircase and no other connections, an old couple lives, one of them with progressive dementia. They ask me what to do if there is a fire in the basement or downstairs apartment.

I don't think even a healthy young person would be able to evacuate safely in that building if a fire started somewhere below. I told them to install a fire door and stay in the apartment in case something happened. Does anyone have any other advice on this?
